Consent to Transfer form (also known as the “Release Paper”)

Employers, please click on the image below to sign the form online and send a copy to Anisya. A copy will also be sent back to the employer. Workers looking to transfer need their current employer to send a signed copy of this form to Anisya before their ad can be activated.

Why do employers need to sign this form?

The Ministry of Manpower requires the FDW’s current employer to sign this declaration to provide consent for her transfer to the next employer. To ensure that future employers on the Anisya website are interviewing FDWs who can indeed transfer, Anisya will activate an FDW’s profile after receiving this signed consent. Some employers have indicated to us their discomfort in signing a half filled form and as an alternative, they can contact us at support@anisya.com, indicating the employer’s name, their worker’s name and work permit number, and mention that they have agreed to the worker’s upcoming transfer.

To promote employment freedom in the FDW sector, Anisya charges zero agency fees to FDWs and there is no obligation on the FDW or prospective employer to use Anisya’s hiring services by signing this form.
